AARP developed this family caregiver guide with you, the caregiver, in mind and as a starting point to help you find the services and support you might need throughout your journey.

Colorado Department of Human Services National Family Caregiver Support Program
Provides services to caregivers assisting older adults, as well as grandparents age 60 and over raising grandchildren.

Colorado Respite Coalition (303-233-1666)
Provides family caregivers access to quality respite care and resources through a network of community partners.

EasterSeals Colorado
Provides support, information and respite for family caregivers.

HeartLight Center (720-748-9908)
Provides affordable education programs for professional and volunteer caregivers working with those who are grieving.



Finances

Colorado Department of Human Services
The Old Age Pension Program provides financial assistance to elderly, low- income residents (60 years of age and older) to help them attain a minimum monthly income.

Colorado Property Tax, Rent, and Heat Rebate Program (303-238-7378)
Provides information for low-income and disabled residents to obtain assistance with their property tax, rent and/or heat expenses.

Colorado Home Modification Tax Credit Program (303-864-7810)
Helps people with an illness, impairment or disability retrofit their residence for greater accessibility and independence.

Low-Income Home Energy Assistance Program
Federally funded program offers low-income households financial assistance to pay for energy costs.

Supplemental Nutrition Assistance Program (800-816-4451 | 303-866-5700)
Eligible households receive food assistance through an Electronic Benefits Transfer card to cover a portion of their food budget, along with nutritional education.

Health

Prescription Assistance Program— Colorado Drug Card Free prescription assistance program helps the uninsured and underinsured gain access to significant savings on prescription medications at local pharmacies.

State Health Insurance Assistance Program (888-696-7213)
Provides one-on-one counseling and assistance to people with Medicare and their families. Offers help with obtaining benefits, filing claims and understanding health care options.

State Medicaid Office — Health First Colorado (800-221-3943)
The Medicaid Waiver program prevents or delays nursing home admission for older adults eligible for nursing home payments by providing medical and support services that enable them to continue living at home.

Legal

Colorado Legal Services
Offers free legal advice, clinics and representation to low-income Coloradans and seniors.

Housing and Care Facilities

Colorado Gerontological Society (855-293-6911)
Provides counseling for older adults and families about long-term care, advance care planning, Medicare and Medicaid. Includes a Housing and Home Care Locator tool.

Colorado Housing Connects (844-926-6632)
Provides access to fair housing resources and answers questions for renters, homeowners, homebuyers and landlords.

Senior Housing Options (303-595-4464)
Provides caring, supportive services and quality affordable housing for older adults.

Sunshine Home Share Colorado (303-915-8264)
Home sharing program helps people above the age of 55 stay in their homes by safely matching them with someone seeking housing

    Provides information on aging- related resources and education for older adults and their families/ caregivers.

    Aging and Disability Resources Center for Colorado (866-959-3017 | 303-480-6700)
    Connects older adults with a broad range of home-based assistance, local information, and government and community-based services. Assistance specialists help find resources and offer referrals to local services.

    ALS Association Rocky Mountain Chapter (303-832-2322)
    Supports people living with amyotrophic lateral sclerosis (ALS) and their loved ones through services and education.

    Alzheimer’s Association: Colorado Chapter (303-813-1669)
    Provides information and support for people living with dementia and their families and caregivers, offering a broad range of free programs and services.

    Colorado Association of Area Agencies on Aging (888-866-4243 | 303-866-2800)
    Offers programs and services created to help support consumers in their homes and communities, including its National Family Caregiver Support Program.

    Office on Aging (720-913-1311)
    Provides information about programs and services for older adults, including an Older Adult Resource Guide.

    Rocky Mountain Stroke Center (303-730-8800)
    Provides information, rehabilitation and support to stroke survivors and their families.

    RTD (303-299-6000)
    Access-a-Ride service offers bus transportation for area residents with disabilities.
